Receiving Pflegegeld

Pflegekasse · Amounts, the advisory visit and combining benefits

InformalFrom Pflegegrad 2

Pflegegeld (long-term care allowance) under § 37 SGB XI is the cash benefit for care provided by relatives, neighbours or friends. It is transferred to your account every month, and you decide freely how to use it – it is neither a wage nor an earmarked refund.

The amounts are set out in the law: €347 for care grade (Pflegegrad) 2, €599 for grade 3, €800 for grade 4 and €990 for grade 5. At Pflegegrad 1 there is no Pflegegeld – other benefits apply at that grade, above all the additional relief allowance (Entlastungsbetrag).

All benefit amounts on this page carry the note “as of 1 January 2025” in the statutory text. They were raised by 4.5% on 1 January 2025 and remain unchanged in 2026; the next scheduled adjustment under § 30 SGB XI is planned for 1 January 2028. That also tells you when it is worth checking the amounts again.

One point is often treated as a formality, and it is not: the advisory visit (Beratungseinsatz). Anyone who draws only Pflegegeld must regularly arrange an advisory visit in their own home – every six months for Pflegegrad 2 and 3, every three months for grades 4 and 5. If it does not take place, § 37 Abs. 6 SGB XI states clearly what follows: the long-term care insurance fund (Pflegekasse) must “appropriately reduce and, in the event of repetition, withdraw” the Pflegegeld.

Required documents

What you need

The notice (Bescheid) on the Pflegegrad

The requirement is an approved Pflegegrad 2 to 5. At Pflegegrad 1 there is no Pflegegeld; at that grade, the Entlastungsbetrag, care aids and grants for improving the home environment are the relevant benefits.

Bank details

The IBAN of the account into which payments are to be made. If an authorised representative or a legal guardian is involved, clarify in advance which account will be paid into – redirecting payments afterwards typically takes weeks.

Choosing the type of benefit

You tell the Pflegekasse whether you want Pflegegeld, benefits in kind (Pflegesachleistung) through a care service, or the combination benefit of both. This choice binds you for six months but can be changed afterwards.

Power of attorney or legal guardian’s ID, if applicable

When relatives are handling the matter. Without a power of attorney, the Pflegekasse gives no information – this is the most common reason for fruitless phone calls.

A reminder for the advisory visit

Enter the dates in your calendar: every six months for Pflegegrad 2 and 3, every three months for grades 4 and 5. The advisory service does not always get in touch of its own accord, and it is the person in need of care who bears the consequences of a missed visit.

Step by step

How to proceed

  1. 1

Choosing the type of benefit — and knowing about the combination

You do not have to choose between cash and a care service. The combination benefit under § 38 SGB XI allows both: if you use the care service only partly, you continue to receive Pflegegeld on a proportional basis. Anyone who uses 40% of the benefit in kind receives 60% of the Pflegegeld.

  2. 2

    Applying for Pflegegeld informally

A single sentence to the Pflegekasse is enough: which type of benefit you want and where payment should go. Payment is made monthly in advance.

  3. 3

    Scheduling the advisory visit

Every six months for Pflegegrad 2 and 3, every three months for grades 4 and 5, carried out by an approved care service, a recognised advice centre or a care professional commissioned by the fund. It costs you nothing and is billed directly to the fund.

  4. 4

Using the advisory visit instead of just ticking it off

It is not an inspection but your best free consultation with a professional: how can transfers be made easier, which aids are you entitled to, when is it worth applying for a higher care grade, how can you avoid becoming overburdened? Collect your questions beforehand.

  5. 5

During respite or short-term care: half continues to be paid

During respite care (Verhinderungspflege) or short-term care, half of the Pflegegeld continues to be paid for the corresponding period. So it does not stop altogether – the fear that it would keeps many people from using this relief at all.

  6. 6

Day care leaves the Pflegegeld untouched

Semi-inpatient day and night care (Tages- und Nachtpflege) is provided under § 41 SGB XI “in addition”, “without this being offset against these entitlements”. Pflegegeld is therefore not reduced.

  7. 7

Check for a higher care grade if the condition worsens

The jump from grade 2 to 3 means €252 more per month, from 3 to 4 a further €201. There is no waiting period for an application for a higher care grade; it is worth filing as soon as the condition has permanently worsened.

Pflegegrad 3€599.00 per month

These amounts are set out directly in § 37 SGB XI and carry the note “as of 1 January 2025” there. So if you come across different figures online, they almost always date from before 2025 – the amounts rose by 4.5% on 1 January 2025 and have been unchanged since. Pflegegeld is tax-free and is paid monthly in advance; you use it freely, and it is neither earmarked nor subject to proof of use. The combination matters: under § 38 SGB XI you can mix Pflegegeld and benefits in kind – anyone who uses the care service only partly continues to receive the corresponding remaining share of the Pflegegeld. And the advisory visit is no minor matter: anyone who draws only Pflegegeld must arrange it – every six months for Pflegegrad 2 and 3, every three months for grades 4 and 5. If it does not take place, the Pflegekasse must, under § 37 Abs. 6 SGB XI, “appropriately reduce and, in the event of repetition, withdraw” the Pflegegeld. It costs you nothing. Incidentally, Pflegegrad 1 also carries an entitlement to advice in your own home every six months – there as a right, not an obligation.

Good to know

Important notes

€347 / €599 / €800 / €990 for Pflegegrad 2 to 5. Unchanged since 1 January 2025, including in 2026.

The next adjustment is due on 1 January 2028. § 30 SGB XI – so you know when it is worth checking again.

At Pflegegrad 1 there is no Pflegegeld, but the Entlastungsbetrag, care aids and grants for improving the home environment are available.

The advisory visit is compulsory if you draw only Pflegegeld. PG 2–3 every six months, PG 4–5 every three months – and it costs you nothing.

Missing it leads to a reduction and, if repeated, to withdrawal. § 37 Abs. 6 SGB XI. Enter the dates in your calendar.

Combination benefit instead of either-or. § 38 SGB XI: anyone who uses the care service only partly keeps the proportional remainder of Pflegegeld.

During respite care and short-term care, half of the Pflegegeld continues to be paid. It does not stop altogether.

Day care is not offset. § 41 SGB XI says so expressly – Pflegegeld remains unreduced.

Avoid these mistakes

Common mistakes

Letting the advisory visit lapse. The most common avoidable reduction for this benefit – and the appointment costs nothing.

Assuming it is “either cash or a care service”. The combination benefit under § 38 SGB XI allows both, on a proportional basis.

Doing without relief because you do not want to lose Pflegegeld. During respite care and short-term care half continues to be paid, and with day care even all of it.

Working from older amounts. Figures below €347 for Pflegegrad 2 date from before 2025.

Not applying for anything at Pflegegrad 1. The Entlastungsbetrag, care aids and home-environment improvements are available at that grade too.

Putting off the application for a higher care grade. There is no waiting period, and the grades differ by two to three hundred euros.

Frequently asked questions

Questions and answers

How much is Pflegegeld in 2026?

€347 for Pflegegrad 2, €599 for grade 3, €800 for grade 4 and €990 for grade 5, each per month. At Pflegegrad 1 there is no Pflegegeld. These amounts are set out in § 37 SGB XI with the note “as of 1 January 2025”: they were raised by 4.5% on 1 January 2025 and remain unchanged in 2026. The next scheduled adjustment under § 30 SGB XI is planned for 1 January 2028. Different figures online almost always date from before then.

What may I use Pflegegeld for?

Whatever you want. Pflegegeld is a free cash benefit: it is not earmarked, it does not have to be accounted for or evidenced, and the Pflegekasse does not ask about it. It is common to pass it on to the relatives providing care or to use it for everyday help, but you are under no obligation to do either. It is also tax-free and is paid monthly in advance. What the fund does check, however, is the advisory visit – not how the money is used, but that home care receives professional support.

What is the advisory visit, and what happens if it is missed?

A regular visit to your home by an approved care service, a recognised advice centre or a care professional commissioned by the fund. It is compulsory if you draw only Pflegegeld: every six months for Pflegegrad 2 and 3, every three months for grades 4 and 5. The consequence of missing it is spelt out expressly in the law: under § 37 Abs. 6 SGB XI the Pflegekasse must “appropriately reduce and, in the event of repetition, withdraw” the Pflegegeld. The appointment costs you nothing and is billed directly to the fund – so there is no reason to let it lapse. Pflegegrad 1 also carries an entitlement to advice every six months, though there as a right rather than an obligation.

Can I combine Pflegegeld with a care service?

Yes, and this is one of the most important and least used options. The combination benefit under § 38 SGB XI allows both to be mixed: if you use the benefit in kind only partly, you continue to receive Pflegegeld on a proportional basis. So anyone who draws on 40% of the in-kind benefit amount through a care service is paid 60% of the Pflegegeld. This is particularly useful for families where relatives take on most of the care, but a service helps, for example, with washing in the mornings. The choice of benefit type is binding for six months and can be changed afterwards.

Do I lose Pflegegeld if I use relief services?

No – and this worry keeps many families from using relief services at all. During respite care and short-term care, half of the Pflegegeld continues to be paid for the duration of the benefit; it does not stop altogether. With day care nothing changes at all: § 41 SGB XI states that semi-inpatient day and night care may be used “in addition”, “without this being offset against these entitlements”. So anyone who uses day care two days a week keeps the full Pflegegeld.

Is Pflegegeld taxed or offset against other benefits?

Pflegegeld is tax-free. Even when it is passed on to caring relatives, it remains tax-free in the usual home-care situation, because it is not earned income – this may be different if someone provides care on a commercial basis. The rules on offsetting against other social benefits differ from benefit to benefit; if you also receive basic income support, Wohngeld (housing benefit) or similar benefits, ask the office concerned specifically rather than assuming a general rule. For caring relatives themselves, compulsory pension insurance may also apply – under certain conditions the Pflegekasse then pays contributions, which in the long run can be worth more than the Pflegegeld itself.

When does payment start?

From the month the application is submitted, as soon as the Pflegegrad has been established – so also retroactively for the period between the application and the official notice (Bescheid). After that, it is paid monthly in advance. The same applies if you move to a higher Pflegegrad: the higher amount is paid from the month of the application for a higher care grade. It is therefore worth submitting both the first application and an application for a higher care grade straight away, rather than waiting until all documents are together – the application is informal and can be made by phone.

What changes when moving to a higher care grade?

The difference is considerable: from grade 2 to 3 it is €252 more per month, from 3 to 4 a further €201, from 4 to 5 another €190. The other benefits also increase – the benefit in kind, day care, the grant for full-time residential care. There is no waiting period for an application for a higher care grade: you can file it at any time, even shortly after a notice, if the condition has permanently worsened. Keep a care diary for one to two weeks beforehand and gather new medical findings – that is the difference between a successful and a refused application.
