Full inpatient care in a care home

Pflegekasse · Grant, personal contribution and Leistungszuschlag

In writingFrom Pflegegrad 2

With full inpatient care, the long-term care insurance fund (Pflegekasse) pays a fixed monthly amount under § 43 SGB XI: €805 for care grade (Pflegegrad) 2, €1,319 for grade 3, €1,855 for grade 4 and €2,096 for grade 5. Even Pflegegrad 1 does not come away empty-handed: there is a grant of €131 a month – an entitlement that is almost never mentioned.

These amounts do not cover the cost of the home. What remains is the personal contribution, and it consists of several parts: the facility-wide uniform personal contribution for care, plus accommodation and meals, investment costs and, where applicable, a training levy.

How high it turns out to be depends entirely on the individual facility and on the federal state. That is why you will find no average figure here: a mean value from a past year tells you nothing about the home you are looking at – the only reliable figure is in the cost estimate of that facility.

What you should know about, however, is the benefit supplement (Leistungszuschlag) under § 43c SGB XI. It reduces the care-related personal contribution as the length of stay increases, and considerably so: by 15% in the first year, 30% after more than twelve months, 50% after more than 24, and 75% after more than 36 months.

What you need

The notice (Bescheid) on the Pflegegrad

From Pflegegrad 2 for the full grant. With Pflegegrad 1, there is an entitlement to a grant of €131 a month towards the expenses – considerably less, but not nothing.

The home's cost estimate

The most important document. It must show separately: the facility-wide uniform personal contribution for care, accommodation and meals, investment costs, and, where applicable, a training levy. Only then can you compare – and only the care portion is reduced by the Leistungszuschlag.

Evidence of income and assets

Not for the Pflegekasse – whose grant does not depend on income – but for a possible application for care assistance under social assistance law (Hilfe zur Pflege) at the Sozialamt (social welfare office), if the personal contribution cannot be afforded.

The home contract

Read it before signing, especially the terms on notice periods, absence rules during hospital stays, additional services and price adjustments. Consumer associations (Verbraucherzentrale) check home contracts – that is time well spent.

Proof of earlier periods of stay

For the Leistungszuschlag, the qualifying months count. If you have already received full inpatient care at another facility, these periods should be taken into account – partial periods count too.

Step by step

How to proceed

  1. 1

Search early and use waiting lists

The search almost always takes longer than expected. Put your name on several waiting lists, even if the move is not yet imminent – registering commits you to nothing. The local care support centre (Pflegestützpunkt) and the long-term care insurance funds’ directories of care homes help you choose.

  2. 2

    Obtain and compare cost estimates

At least two to three. Compare item by item: care portion, accommodation and meals, investment costs, training levy. The differences between homes in the same town are often bigger than expected, and only the care portion is later reduced by the Leistungszuschlag.

  3. 3

Apply to the Pflegekasse for cost cover

The Pflegekasse pays the amount for your Pflegegrad directly to the facility. It is independent of income – only the Pflegegrad is checked.

  4. 4

Keep an eye on the Leistungszuschlag

Under § 43c SGB XI, the care-related personal contribution falls as the length of stay increases: 15% up to twelve months, 30% after that, 50% after 24 and 75% after 36 months. As of 1 July 2026, the Pflegekasse calculates it based on information from the facility.

  5. 5

    Check whether earlier periods count

Partial periods and time spent at other facilities also count towards the qualifying months. If you move, or if you have received full inpatient care before, point this out – otherwise the scale may start again from zero.

  6. 6

If the personal contribution is unaffordable: care assistance

If your income is not enough, care assistance from the Sozialamt may be an option – not from the Pflegekasse. Apply early; retroactive benefits are only possible to a limited extent.

  7. 7

    Have the home contract checked

Before you sign. Pay attention to the rules on absence during hospital stays, notice periods, and which services are charged extra. Consumer associations offer a check for this.

The grants from the Pflegekasse are set out in § 43 SGB XI with the note “as of 1 January 2025” and remain unchanged in 2026: €805, €1,319, €1,855 and €2,096 for Pflegegrad 2 to 5. Figures such as €770, €1,262, €1,775 and €2,005 date from before then. Not to be forgotten is Pflegegrad 1: there is an entitlement there too, namely to a “grant of €131 a month” – a point that is almost always missing from overviews. These amounts are independent of income and do not cover the cost of the home. The personal contribution consists of the facility-wide uniform care portion, of accommodation and meals, of investment costs and often a training levy. How high it is depends on the individual facility and on the federal state – which is why there is deliberately no average figure here: a mean value from a past year tells you nothing about the home you are looking at, and also quickly becomes out of date. The only reliable figure is in the facility's cost estimate, broken down by item. The personal contribution can be considerably reduced through the Leistungszuschlag under § 43c SGB XI: it amounts to 15% of the payable care-related personal contribution for up to twelve months, 30% after more than twelve, 50% after more than 24 and 75% after more than 36 months – for Pflegegrad 2 to 5. As of 1 July 2026, the Pflegekasse calculates it based on information from the facility. Partial periods and time at other facilities also count towards the qualifying months. If the personal contribution is still not affordable, care assistance from the Sozialamt is the right route – not from the Pflegekasse.

Good to know

Important notes

€805 / €1,319 / €1,855 / €2,096 for Pflegegrad 2 to 5, unchanged since 1 January 2025.

Pflegegrad 1 gets something too: €131 a month as a grant in full inpatient care. Almost never mentioned.

The grant is independent of income. What is checked is the Pflegegrad, not your assets.

The personal contribution falls with duration: 15% up to 12 months, then 30%, 50% and 75% – § 43c SGB XI.

As of 1 July 2026, the Pflegekasse calculates the Leistungszuschlag based on information from the facility.

Earlier periods of stay also count, including partial periods and periods at other homes. Point this out.

An average figure does not help you. Request the cost estimate with separately shown items.

You apply for care assistance at the Sozialamt, not at the Pflegekasse – and as early as possible.

Avoid these mistakes

Common mistakes

Calculating with the old amounts. €770 instead of €805 for Pflegegrad 2 – the figures from before 2025 are still everywhere.

Not applying for anything at all with Pflegegrad 1. The grant of €131 a month in full inpatient care is almost always overlooked.

Only comparing the total sum. Without separate items you cannot see which part is reduced by the Leistungszuschlag.

Not declaring earlier periods in a home. The scale under § 43c may then start over, even though it should continue.

Planning with an average figure from the internet. It is outdated and says nothing about the specific facility.

Applying for care assistance too late. Retroactive benefits are only possible to a limited extent – and the bill keeps running.

Frequently asked questions

Questions and answers

How much does the Pflegekasse pay in a care home?

A fixed monthly amount under § 43 SGB XI: €805 for Pflegegrad 2, €1,319 for grade 3, €1,855 for grade 4 and €2,096 for grade 5. In the law, these amounts carry the note “as of 1 January 2025” and remain unchanged in 2026. There is also something for Pflegegrad 1 that is almost never mentioned: a “grant of €131 a month”. The grant is independent of income and is paid directly to the facility. Figures of €770, €1,262, €1,775 and €2,005 date from before 2025.

What does a care home place cost altogether?

We cannot tell you, and no one can do so reliably – which is why there is deliberately no average figure here. The personal contribution is made up of the facility-wide uniform care portion, of accommodation and meals, of investment costs and often a training levy. These items differ considerably between facilities in the same city, and even more between federal states; a mean value from a past year tells you nothing about the home you are looking at and quickly becomes out of date. Ask for the cost estimate with separately shown items and compare two to three homes item by item – that is the only calculation that is right for you.

Is it true that the personal contribution falls over time?

Yes, and considerably so. § 43c SGB XI grants people in need of long-term care with Pflegegrad 2 to 5 a Leistungszuschlag on the payable care-related personal contribution: 15% for up to twelve months, 30% for more than twelve months, 50% for more than 24 and 75% for more than 36 months. What matters is the reference value: only the care portion is reduced, not accommodation, meals and investment costs. Partial periods and time at other facilities also count towards the qualifying months. As of 1 July 2026, the Pflegekasse calculates the Leistungszuschlag based on information from the facility – if it is missing from your bill or appears incorrect, the Pflegekasse is your point of contact.

What if we cannot pay the personal contribution?

Then care assistance may be an option – applied for at the Sozialamt, not at the Pflegekasse. It is a social assistance benefit that takes income and assets into account; maintenance claims against children can also play a role, though only above a substantial income limit, so most children are not required to pay. Apply early: retroactive benefits are only possible to a limited extent, while the home's bill keeps running. Get advice beforehand – care support centres and social associations know the procedures and help with the application free of charge.

Will a care home place be paid for with Pflegegrad 1?

Only to a small extent – but not nothing. § 43 SGB XI provides a “grant of €131 a month” for people in need of long-term care with Pflegegrad 1 in full inpatient care. That is considerably less than from grade 2 onwards, but it is almost always left out of overviews – many assume there is nothing at all in a care home with grade 1. If a care home place is coming up and the condition has worsened, it is also definitely worth submitting an application for a higher care grade: the difference between €131 and €805 a month is considerable, and there is no waiting period.

What must I watch out for in the home contract?

Four points deserve particular attention. First, the absence rules: what applies during a hospital stay, how long is the place kept open, and what is charged during this time? Second, the notice periods – for both sides. Third, the additional services: what is included in the price and what is charged separately, such as a hairdresser, chiropody or special room fittings? And fourth, the price adjustment: under what conditions may the facility raise prices, and with what notice period? Consumer associations check home contracts; given the amounts involved, this cost is money well spent.

Do periods at another home count too?

Yes. For the Leistungszuschlag under § 43c SGB XI, what matters is the number of qualifying months of full inpatient care, and partial periods are taken into account too. If you move – for example to a home closer to relatives – or if there was already a period of full inpatient care before, these periods should feed into the scale and the scale should not start again from zero. Point this out explicitly when you change homes and submit proof of the earlier periods. As of 1 July 2026, the Pflegekasse calculates the Leistungszuschlag based on the facility's information; otherwise a transmission error at the time of the move goes unnoticed by everyone except you.

Can I use Pflegegeld (long-term care allowance) or the additional relief allowance (Entlastungsbetrag) to pay towards it?

No, neither is available in full inpatient care. The Pflegegeld under § 37 SGB XI presupposes home care and is not paid during a permanent stay in a home; likewise, the Entlastungsbetrag under § 45b SGB XI is reserved for people in need of long-term care “receiving home care”. In full inpatient care, their place is taken by the grant under § 43 SGB XI plus the Leistungszuschlag under § 43c. The position is different for a temporary stay: during short-term care, half of the Pflegegeld continues to be paid, and the Entlastungsbetrag may be used for it.
